Discharge Form Corrected

Otis Davis, a Vietnam veteran, was missing the Army Commendation Medal with Valor device from his discharge form.  Orders found, a certificate was issued and his discharge form was corrected.

His citation reads:

“For heroism in connection with military operations against an armed hostile force in the Republic of Vietnam. Private First Class Davis distinguished himself while serving as a cannoneer with Battery A, 6th Battalion, 29th Artillery, 4th Infantry Division. During the period 30 October to 31 October 1968, Private First Class Davis’ firebase was subjected to intense heavy weapons fire. Despite the intense barrages of enemy fire impacting inside of the firebase, Private First Class Davis remained at his howitzer and assisted in placing accurate counterfire on the enemy positions. The effectiveness of these counterfires were largely responsible of the neutralization of the enemy firing position. Private First Class Davis’ personal bravery, initiative and exemplary devotion to duty are in keeping with the highest traditions of the military service and reflect great credit upon himself, his unit and the United States Army”
